For independent inspectors

The inspection form you'll complete

This is a read-only copy of the exact form Autoquiry inspectors fill out on every private-vehicle Full inspection. Nothing here saves or submits — scroll from top to bottom so you can see every section, every note field, every photo requirement and the final assessment before you agree to take assignments.

What you must record on every inspection

  • 28 checklist points across 7 categories, each rated OK, Minor, Major, Critical or N/A.
  • Written notes on every item — an item is not complete without them.
  • At least one photo in every section, except Diagnostics & History.
  • One cover photo and one walk-around video of the whole vehicle.
  • Repair estimates with costs in USD — critical and preventative.
  • 33 Full report detail fields, of which 32 need a comment before you can submit.
  • Overall condition, risk level, purchase recommendation and your own 0–100 score.
  • Odometer, inspection location and a plain-language overall summary.

If a check cannot be performed, you must record the limitation rather than assume or report that the item passed.

Fields marked are required.

Vehicle photo for report cover

Start here: take one clear photo of the whole vehicle (front three-quarter view, in good light, nothing blocking it). It is shown on the cover of every report for this car.

Walk-around video

Record one steady walk-around of the whole vehicle (about 1–2 minutes, all four sides, roof line and wheels, in good light). It is shown only in the Full report.

28 of 28 items must be recorded before you can submit. The form opens with the vehicle's year, make, model and VIN already filled in from the order.

Risk Level Definition

Low Risk
No significant issues identified during the inspection.
Moderate Risk
Issues identified that may require repairs, maintenance, or further evaluation.
High Risk
Significant issues identified that could lead to substantial repair costs or affect the vehicle’s suitability for purchase.
Indeterminate
Too little could be inspected to judge risk reliably.

Important: The risk level reflects the issues actually identified, not a guarantee that no hidden problems exist.

Purchase Recommendation

RECOMMENDED
The inspection findings support proceeding with the purchase, subject to price and buyer preferences.
PROCEED WITH CAUTION
Consider purchasing only after evaluating the identified issues, potential repair costs, and asking price.
NOT RECOMMENDED
Significant issues make proceeding inadvisable without further evaluation.
INSUFFICIENT INFORMATION
Inspection access or results were too limited to make a recommendation.

Finish and submit

Submitting publishes your inspection report automatically and gives the ordering customer access to their Full report. Autoquiry may perform an optional quality check afterward.

Until every required item above is filled, the form lists what is still missing and takes you straight to it.
